Study finds doctors have exaggerated fears when starting patients on insulin

TORONTO, Ont. April 2, 2012 Doctors are more reluctant to start their patients on insulin than the patients are themselves, according to a new study led by researchers at St. Michael's Hospital.

"It is well-known that insulin is effective in lowering blood sugar," said Dr. Catherine Yu, a researcher at the hospital's Keenan Research Centre and senior author of the paper. "But there are no clear recommendations on the safest and most effective way to start patients on it, and so physicians are often hesitant to do so."

Dr. Yu and colleagues analyzed past studies to find out what barriers existed to starting patients on insulin, and how insulin compared to other blood sugar lowering medications in terms of its effect on blood sugars and weight. They then made recommendations for physicians and other health care providers based on evidence from the past studies.

Their findings were published in today's online edition of the Canadian Medical Association Journal.

They found that doctor's fears of common side effects such as weight gain and low blood sugar were amplified compared to their patient's fears, and that doctors were also more concerned than their patients about the possibility of injection-related pain and anxiety.

"When some physicians think of the side effects of insulin and the barriers to starting it, they're often thinking about the older types of insulin and older delivery systems," Dr. Yu said. "So a lot of the hesitation may be that their way of thinking about the drug has not changed along with the new types of insulin that are used."

She said better insulin is now used in smarter ways, and that it acts more like the insulin we manufacture in our bodies resulting in less risk for low blood sugar and weight gain.

"Everyone knows that insulin works, but the key point here is that it's safe and can be straightforward to use too," Dr. Yu said.

When Dr. Yu and colleagues compared the best ways to start patients on insulin, they found one insulin injection a day was just as effective as other regimens, caused less severe side-effects and was more manageable for physicians and the health care team.

Patients on one injection a day therapy typically gain about 2.2lb.-3.3lb. less than those receiving insulin twice a day or multiple times a day, which each average 0.6lb-14lb. in weight gain.

For patients already on pills to lower their blood sugar that also need insulin, combination therapy continuing the oral medication when starting insulin is more effective in eliminating side effects and requires a lower insulin dose for the same effectiveness.

Dr. Yu warns it's important to watch out for the type of needle that's used. She published research last month showing that using safety insulin needles with retractable shield often used to protect health care providers should be used with caution. In some patient types, delivery of insulin can be inconsistent, leading to unpredictable blood sugar levels and an increased risk of diabetic emergencies.

Former Apopka High football player Bobby Henderson arrested in sex assault in S.C.

Former Apopka High School football player Bobby Henderson is one of two college students arrested Friday in the sexual assault of three women in an off-campus student apartment in South Carolina, authorities said.

Henderson, 19, a Presbyterian College football player from Pine Hills, was arrested on charges of burglary, kidnapping and sexual assault in an attack that police say happened the morning of March 21 at Bearcat Village, a half-mile from Lander University in Greenwood, S.C.

Cameren Jones, 19, of South Carolina, another Presbyterian College football player, also was arrested Friday in the case.

According to the university, the two men forced their way into the apartment at gunpoint. One of the women previously had an unspecified relationship with Jones, Lander police said.

Presbyterian College officials said the students have been suspended. They were being held Saturday in the Greenwood County Detention Center without bail.

Vaser for Men: Is It Effective? - Women's Health

Several men endure liposuction every year so that they can accomplish a sculpted figure by losing some extra pounds. However, the fact is that the effects of liposuction tend to differ in men than in women. Nevertheless, men might experience better results with the Vaser Lipo treatment.

What Experts Say on Vaser Lipo for Men

According to the cosmetic surgeon, Dr. Paul M. Parker; the accumulation procedures in men are different than those in women. Physically, women gain more fat around their thighs, hips, and stomach; whereas men accumulate fat in the chest area, love handles, and atop the abdominals. Because of the high density of fat in these areas, men experience better results when they undergo the Vaser Liposuction treatment, a procedure that transmits the ultrasonic energy waves to the tissues for fat break up and extraction.

What Men Can Achieve with Vaser Lipo

Better result is quite a subjective term to understand what Vaser Lipo can make you achieve. Translating it objectively, this new technique promises to offer the sculpted six-pack abs apart from good looks, slimmer body areas, and natural lightweight feeling. For men, Vaser is the minimal invasive technique for removing excess fat from around their love handle and chest areas. The biggest benefit is natural-looking results that a conventional liposuction treatment does not provide.

For Which Body Parts is the Vaser Lipo Treatment Performed

In men, the Vaser Lipo treatment is performed on the neck and face areas for discarding additional fatty deposits from around the jawline, love handles, back, abdomen, and on the area affected by gynecomastia. Treatment of gynecomastia before Vaser was painful, needed several weeks of downtime, and induced scars. However, with the Vaser Lipo treatment, you can enjoy permanent and virtually painless procedure for treating this condition, as the no surrounding tissue is disturbed during the process.

How the Treatment is Performed

Usually, the cosmetic surgeon performs it under a local anaesthetic after which ultrasonic energy is transmitted to the tissues and a fine probe is inserted for emulsifying the fats.

What?s the Treatment Duration and Recovery Time?

Above all, it takes less than two hours for the treatment to get over and that you can return to work on the second day. However, this will depend on the amount of fat extracted and the number of areas hit.

What are the Side Effects or Complications Involved

Fortunately, they are minimal. Little risk of scarring, swelling, and bruising is involved and that you need to wear the support garments for two weeks. Further, there is no risk of weight gain if you concentrate on suggested exercises and diet plans.

How Do I Choose an Ideal Vaser Lipo Surgeon?

Always choose a board-certified liposuction doctor who has good experience in performing this treatment (check this out by customer reviews online). Otherwise, improper handling of probe might invite injuries. Second, compare the liposuction cost and experience (in years) of the various cosmetic surgeons according to the area. For instance, the liposuction cost in a liposuction clinic London for one area is around ?2,800. In case of four areas, the cost of liposuction in London is around ?4,500. However, you can expect discounts for Vaser Lipo in London.

'Weird Al' Yankovic Suing Sony Music for $5 Million

Jason Merritt/Getty Images

"Weird Al" Yankovic, through his company Ear Booker Enterprises, is suing Sony Music Entertainment for $5 million.

The complaint alleges that Sony Music took improper and duplicate recoupments, which resulted in royalty under payments. It also alleges the major is playing Yankovic a straight royalty for downloads sales instead of the 50% of revenues that a licensing deal calls for. Additionally, it says Sony Music hasn't shared any money it received from lawsuit settlements from such cites as Napster, Kazaa and Grokster.

The lawsuit states that Sony Music received an equity stake in YouTube in exchange for providing the site with the company's content. At the time, Sony Music was given an equity stake; Yankovic "White & Nerdy" video was among the most popular content on the site. The suit says, "a portion of Sony's equity share in YouTube is directly apportionable and allocable to "White & Nerdy," as well as other content created by Yankovic.

The lawsuit was filed in Federal court in the Southern District of New York by the New York based firm Gordon, Gordon & Schnapp and the Nashville-based firm King & Ballow.

Richard Busch, a partner with the Nashville-based firm has successfully sued Universal Music Group for treating downloads as a sale instead of a license and now the major has to pay Eminem's former production company, F.B.T. Productions, 50% of net fees from download sales. He has since filed lawsuits on this point on behalf of Peter Frampton, Kenny Rogers and the estate of late Knack drummer Bruce Gary.

Sony Music could not be reached for comment at deadline.
